We work with every new patient to explain what to expect, answer questions, and prepare for a thorough assessment. What do I need to bring to my first appointment?
When you are a new patient, it is important that we gather as much information as possible regarding your ocular history, general health and family history. Please complete our Welcome to For Eyes Optometrist form (see links below) and bring with you to your first appointment.

Getting to Our Clinic and Finding Parking

Our store is located on High Street, with 1-hour free parking bays along both sides of the road. This should allow enough time for your eye test.

2-hour parking: Parry Street (EasyPark location 34134)
All-day parking:
Ellen Street near the tennis courts (EasyPark location 34551)
Fremantle Prison parking facility (EasyPark location 34549) off Parry Street

And our clinic is 650m away from Fremantle Station should you choose the Fremantle train line
Important to note: the Fremantle Traffic Bridge is being replaced with a new gateway to Walyalup (Fremantle). While construction techniques are allowing the existing bridge to stay open for the remainder of 2025, the bridge will close for up to 12 months starting in early 2026.
